Why it may help
Glaucoma involves elevated pressure inside the eye (intraocular pressure) that can gradually damage the optic nerve. The eye contains components of the endocannabinoid system, including CB1 receptors in tissues that regulate fluid drainage and pressure, such as the ciliary body and trabecular meshwork. Researchers believe cannabinoids like THC may interact with these receptors to influence how aqueous fluid is produced and drained, which is the pathway through which cannabis has been observed to temporarily lower eye pressure.
How patients use it
The main effect patients pursue is a reduction in intraocular pressure, since lowering eye pressure is the central goal of glaucoma management. Research suggests that cannabis may lower eye pressure, but the effect appears to be short-lived, generally lasting only a few hours, which would require very frequent dosing to maintain. Because of this short duration and the side effects of repeated dosing, leading ophthalmology organizations do not currently recommend cannabis as a substitute for established glaucoma treatments.
Strength of the evidence
The evidence for cannabis in glaucoma is limited: while studies have shown it can briefly lower intraocular pressure, the effect is too short-lasting to make it a practical or recommended treatment compared with standard therapies.
